Sextant #3 Buccal Aspect

 

 

Advanced instrumentation fulcrums on sextant #3 from the buccal aspect includes three reinforcement techniques.  All of these techniques are for the right handed operator, however they would only need to be reversed for the left handed operator.  The first fulcrum is the left thumb bridging on the right thumb with the index finger in the vestibule. The second fulcrum is the left index finger on the upper shank.  The third fulcrum is the pinching technique.  Using the Gracey 11/12 and 13/14 mini-after-fives with a vertical stroke interproximally plus these reinforced fulcrums will give the added lateral pressure needed to remove tenacious calculus.  The Gracey 7/8 mini-after-five will use the identical fulcrums with the oblique stroke used instead of a vertical stroke on the buccal and lingual surfaces. It is highly recommended when there is furcation involvement to use reinforced fulcrums for stability and control.
